Baptism Request Form

Mahalo for your interest in being baptized. Baptism is a tremendous blessing and an important first step of obedience for new followers of Jesus. In this form, we will walk you through the following steps, with the last step being the actual request form. 

1. Requirements for Baptism

Baptism is one of the most significant experiences a Christian will have in life. We want it
to be as meaningful for you as God intended.

For this reason, we require each person baptized by our church to affirm these statements below wholeheartedly.

Please read and affirm each statement that you personally agree.

2. The Significance of Baptism

“What shall we say then? Are we to continue in sin that grace may abound? By no means! How can we who died to sin still live in it? Do you not know that all of us who have been baptized into Christ Jesus were baptized into his death? We were buried therefore with him by baptism into death, in order that, just as Christ was raised from the dead by the glory of the Father, we too might walk in newness of life.” (Romans 6:1-4)

WHY BE BAPTIZED?
To identify with Christ in his death and resurrection.
- “Having been buried with him in baptism, in which you were also raised with him through faith in the powerful working of God.” (Colossians 2:12) To signify our acceptance of his sacrifice alone to pay for our sins.

- “He himself bore our sins in his body on the tree, that we might die to sin and live to righteousness. By his wounds you have been healed.” (1 Peter 2:24) To symbolize the death of our old sinful selves, which brought us new life in Christ.

- “I have been crucified with Christ. It is no longer I who live, but Christ who lives in me. And the life I now live in the flesh I live by faith in the Son of God, who loved me and gave himself for me.” (Galatians 2:20) To show the world that physical death has no power over us.

- “O death, where is your victory? O death, where is your sting?" (1 Corinthians 15:55)


THE ONGOING SIGNIFICANCE OF BAPTISM
This is what it means to be a Christian - to live in the reality of what our baptism portrays: day by day we look away from ourselves to God and say, "Because of Christ, your Son, I come to you. In him I belong to you. I am at home with you. He is my only hope of acceptance with you. I receive that acceptance anew every day. My hope is based on his death for me and my death in him. My life in him is a life of faith in you, Father. Because of him I trust your working in me and for me. The same power and glory that you used to raise him from the dead you will use to help me. In that promise of future grace I believe, and in that I hope. That is what makes my life new. O Christ, how I glory in what my baptism portrays! Thank you for dying my death for me and giving new life to me. Amen." [John Piper]
